Requesting Your Medical Records

If you are a former patient and require a copy of your medical records, please contact our medical records department:

You may submit your authorization for release of medical records through either method.

Delivery Options and Fees

•     Mail Delivery: Records can be mailed to you via Priority Mail for a $35 fee.
•     Encrypted Email Delivery: Records may be sent as an encrypted PDF at no charge.
Please note that medical records will be retained until February 28, 2030.

Micro-laparoscopy

While a standard laparoscopic incisions measure 1/2 to 1.2 cm in size, recent developments in surgical instruments make it possible to perform surgery through a needle sized incision (1-2 mm).

Aside from cosmetic benefits, this results in much less incisional pain after surgery, decreased need for pain medication after surgery, and a faster return to daily activities.
